Position Summary

The Community Support Therapist at CLA works with clients to support them achieve their highest possible level of independent functioning, stability, and facilitate recovery. This position is responsible for providing direct clinical therapy services to clients who are experiencing mental health, co-occurring conditions, and substance abuse recovery.

CLA has a strong and highly-supportive Community Wellness Solutions Team that works together to meet the needs of our clients, their families, and our staff. Employees get to provide services to those in need while also enjoying an AMAZING work-life-harmony! This position requires work to be completed in client home, in our office, and the possibility of completing some work remotely once job-competency has been demonstrated.

Benefits:

  • Pay starts at $35/hr based on experience

    • FT - Mon-Fri, 8:30AM-4:30PM, 37.5 Hours Per Week, NO WEEKENDS
    • PT - Mon-Fri, 20-29 Hours Per Week, NO WEEKENDS

*Must work an average of 30 or more hours a week

Primary Duties:

  • Provide ongoing counseling and support to assigned clients.
  • Create and manage a treatment/assessment plan.
  • Collaborate with client support teams to address unmet long term care needs.
  • Utilize effective therapeutic strategies to engage program participants, enhance motivation, and encourage meaningful, collaborative, and strengths-based goal planning and progress.
  • Provide conflict resolution/mediation, lifestyle coaching, support, and advocacy with individuals.
  • Assistant client with identifying and establishing natural and professional supports in the community.
  • May provide Substance Abuse Professional (SAP)/Consultation services for Comprehensive Community Services (CCS) clients. May provide consultation and education to CLA team members and providers working with consumers.

Minimum Education/Experience and Requirements:

  • Master's Degree in Mental Health, Human Services, or related field, required.
  • Current license as a LCSW, LPC, LMFT, or other related professional certification in the State of WI, required.
  • 2 years SAP experience, preferred.
  • 2 years of related experience providing community based mental health services and knowledge of individuals with physical and developmental disabilities and other chronic conditions, required.
  • Comprehensive Community Services (CCS) credential in Dane County WI, preferred.
  • Valid driver's license, reliable transportation, and current auto insurance to serve clients throughout Dane County, required.
  • Must be at least 18 years or older.
  • Must be authorized to work in the United States.
